Patron Interactions

Use this form to record information transactions, information consultations, directional transactions, and operational transactions that occur anywhere, anytime, unless you are staffing a public desk.

If your personal desk doubles as your department's public desk, you can record those transactions on this form.

How did the patron reach you? - Phone, IM, etc.
Transaction involved
Was the transaction with an Extension or distance learning patron? Was it part of a liaison activity?
Patron Status
Is the patron a student, faculty, staff, etc.?
Academic College
To which college does the patron belong? (if known)
Academic Department
To which department does the patron belong? (if known)
Other Academic Group
If the patron is affiliated with another group - a research center or administrative office, etc. - note it here.
